Location of event: Alexandria River Front Center (707 2nd St, Alexandria, LA) , St. Francis Xavier Cathedral (626 4th St., Alexandria, LA) and the Holiday Inn Downtown (701 4th St., Alexandria, LA). Departure and arrival time information will be sent directly to guardians. Hotel accommodations are at Holiday Inn Downtown.

Every activity sponsored by St. Theresa of Avila Parish is carefully planned and adequately supervised by adults. However, even with the best of planning and precaution, unforeseen events can occur. By signing this form, the parent or guardian agrees to assume and accept all risks and hazards inherent in all church-related religious education activities. Either parent, if both parents have legal custody, or the parent or person having legal custody or the legal guardian, of the minor agree not to hold St. Theresa of Avila and the Diocese of Baton Rouge, its employees or volunteer assistants liable for damages, losses or injuries to the person or property undersigned. They also agree that the minor child (as named above) is medically capable of participating in this event. In the case of a simple injury, I authorize the supervisors to administer/obtain basic first aid (Tylenol, acetaminophen, ibuprofen, cough drops, as well as Neosporin and band aides). In the case of a medical emergency, the supervisor will contact the parent/guardian or the person listed as the emergency contact to seek competent, emergency assistance as soon as possible even if that means calling an ambulance to bring them directly to the emergency room of the nearest hospital. The parents or guardians understand that they are signing for the minor listed on this form and the signature is for both a medical and liability release. This authorization shall be effective ONLY for the day(s) of the event.

Group will travel to and from the event by charter bus. Departure time and location information will be sent directly to parents/guardians prior to event.
We expect your son/daughter to conform to these rules of conduct: no possession or use of alcohol, drugs, or tobacco; no fighting or use of a weapon; no offensive or immodest clothing; participating and staying with the group is expected; respect for one another as well as adult leadership; respect and comply with the schedule of the event as established by the facilitator or leadership. Students who fail to comply with any of the expectations will contact their parent/guardian to come and pick them up.
